This end-of-project webinar summarised the results of five Prevention Centre research studies exploring the important role of Primary Health Networks (PHNs).

We need to plan for implementation of the guidelines if we are to improve the uptake of evidence-based CVD risk assessment and management. The project collaborated with stakeholders such as the Heart Foundation, the Royal Australian College of General Practitioners (RACGP) and software providers to disseminate the project’s findings and connect stakeholders in support of the new guidelines.
